Insurance Information

Dental insurance coverage has become more complex due to the vast variety of plans available, and the range of benefits varies from one plan to another. Some plans may pay on a fee schedule which is unrelated to our office fees. We are a Delta Dental of Wisconsin Premiere provider.

Please bring your insurance information with you to the initial visit. This includes the insurance company name and address, group#, subscriber ID#, and subscriber's birthdate. This will expedite processing your insurance claim. When treatment is started, we will ask for an estimated insurance deposit. After payment has been received from the insurance company, if there is any remaining balance, a bill will be sent to you. If for any reason the dental claim is rejected, you are responsible for payment of all fees for services rendered.
